A plan reassessment is when the NDIA replaces your current plan with a new one. It happens when your reassessment date is coming up, or when your situation has changed significantly and your plan no longer meets your needs. A little preparation makes the conversation easier and helps your next plan reflect what you actually need. This guide walks through the steps the NDIA describes on its How to prepare for a plan reassessment page.

Plan variation or plan reassessment?

The NDIA can change a plan in two ways. A plan variation is a small change to part of your current plan, such as updating your goals, your reassessment date or the way your funding is managed. A plan reassessment is a significant change, where the NDIA creates a new plan for you. You can read more about both on the NDIA's guide to changing your plan.

Step 1: Expect a check-in before your reassessment date

According to the NDIA, your my NDIS contact will get in touch about 2 to 3 months before your current plan's reassessment date for a check-in. They'll ask how you're going with your current plan and whether your situation has changed in the last year. If you need a plan reassessment, they'll start the process within 21 days, and explain what information and evidence you'll need and how the process works.

Step 2: Think about the next few years

The NDIA suggests thinking about any life changes you expect over the next 3 years, such as leaving school, starting or leaving work, or a change in your living situation. It also suggests thinking about how connected you are with your social and community supports, and your goals and preferences about how long your plan should run. Jotting these down before your check-in means you won't forget anything on the day.

Step 3: Gather information and evidence

The NDIA can only include supports in your plan that its rules say it can fund, so evidence matters. Evidence might include assessments or reports from your treating healthcare professionals, therapists or support workers, showing how your supports are helping you work towards your goals. These reports can also recommend supports you may need in future. If you're asking for a reassessment yourself, the NDIA says evidence should usually be recent: within 4 months of your request, and after your current plan was approved.

Step 4: Send your information to the NDIA

You can give the NDIA your information and evidence through its service hub, by mail to NDIA, GPO Box 700, Canberra ACT 2601, or in person to your my NDIS contact or a local office. If you're asking for a reassessment outside a regular check-in, the NDIA says the request must come from you, your plan nominee or your child representative on the plan reassessment request form. A provider can't ask on your behalf, although we can help you pull your information together.

Step 5: After your reassessment

An NDIA planner reviews your information and considers which supports are related to your disability, will help you work towards your goals, increase your independence, and help you work, study and join social activities. The NDIA says it will let you know its decision within 21 days. There are three possible outcomes: your plan is varied, a new plan is created, or your plan isn't changed. If a new plan is created, the NDIA will book a plan meeting with you. You can keep using your current plan while the NDIA decides whether to reassess it.

Budget changes that apply at reassessment

From 1 October 2026, changes to the Social, Economic and Community Participation and Improved Daily Living Skills budgets apply as plans are created or reassessed. The NDIA says the impact will be different for every participant and some supports in those categories aren't affected. Read the details on the NDIA's Changes to support budgets from 1 October page, and talk to your my NDIS contact or Support Coordinator about what it means for you.
